I am pleased to announce that effective January 1, 2010, Mature Health Choices has become acquired by Constellation Health Services and is now known as Constellation Geriatric Care Management.  There are offices in Norwalk and Bridgeport, CT ...and an additional branch is anticipated to open in Orange, CT in Spring 2010. 

Constellation Geriatric Care Management currently serves Fairfield and New Haven Counties in CT.  I am excited about joining a bigger team which will benefit my past, current and future client/families and professionals working with them to navigate the long term care system and remain in charge of their lifestyle, living as independently as possible.  Also, I will have greater opportunity to educate professionals and the community about the value and benefits of working with our team of Certified Care Managers.
